Intro:
The Patient-Centered Primary Care Collaborative of Central Ohio is comprised of primary care teams , health plans, employers, and other strategic partners.
The patient centered medical home (PCMH) is an approach to provide comprehensive primary care to patients. PCMH create partnerships between patients and providers, while increasing access and overall value of care.
